Patient Forms

As part of your initial treatment visit we will have a series of papers for you to fill out. Most of these forms are required by your insurance company. Please either bring the appropriate information and arrive at least 20 minutes early or you may download the paperwork here and save some time. You should still plan on arriving early. There will be other forms that are specific to you and your insurance coverage that you will want to review.

What to Expect for Your First Appointment

  1. Please arrive at least 20 minutes before your scheduled appointment time. There are some forms and paperwork that typically needs to be completed before the physical therapist can start your evaluation. You can start your paperwork at home to save you time at your appointment by clicking on the patient forms tab above, printing out your paperwork and completing it before you arrive.
  2. The physical therapist will sit down with you and perform a thorough evaluation of your injury. They will ask you several questions pertaining to your medical history and perform specific functional assessment tests to determine how to best help you.
  3. After the physical therapist has evaluated your injury they will create a treatment plan with you customize it to your specific situation and discuss some treatment goals with you.
  4. Treatment is likely to begin at your first treatment.
  5. Please wear appropriate clothing so that the affected body part can be accessed easily. For example, if you have a knee injury, bring or wear shorts rather than long pants.
  6. You will be able to make follow up appointments at the time of your treatment based on the goals and frequency discussed with the physical therapist.
  7. Your appointments will typically last about 60 minutes.

What to Bring With You

Please bring the following items with you to your first appointment:

  1. Your health insurance card. We will make a photocopy of it. This will allow us to bill your insurance your insurance properly on your behalf.
  2. If you are being treated for a worker’s compensation injury, please have information regarding your claim such as your claim number, adjustor’s name and contact information.
  3. Physician’s referral / prescription for physical therapy services. We are required to have a referral regardless of whether your insurance company requires it for services to be performed.
  4. Co-payment if applicable. Co-payments are due at the time of service and will be collected before your treatment session. We will verify your benefits, including co-payment amounts as a courtesy at the start of your care. Please note we accept cash, check and Mastercard/Visa. We also have a “quick pay” option for your convenience.
